Various forms are required when requesting documentation from the Registrar's/Record's office. Follow the link below to access the appropriate Adobe Acrobat file. - Obtain Good Student Status for Insurance Purposes-if you can get an insurance break by verifing that you are a good student, you can obtain this verification through the UASOM Registrar's Office.
- Obtain Letter to Bank/Loan Institution Verifying Student Status-to be able to defer your loans while you are in school, your bank or loan institution will usually require you to have the school verify that you are a current student.
- Medical Licensure Form/Verification of Graduation-when preparing for graduation and beginning your residency work, you will need the UASOM to verify that you have indeed graduated from the school.
