I can't find my IRB training certificate, so can I get another copy?
Yes—the OIRB can email you a copy of your own certificate. Send your request by e-mail to lgriner@uab.edu. It may take a few days to fulfill your request.
The OIRB charges a fee of $20 per certificate to provide copies of other people's IRB training certificates, as noted in a memorandum of September 9, 2002.
If the only reason you need the certificate is to prove that you completed IRB training for a grant, you may be able to use the OGCA's form to certify everyone on the project. That form and instructcions for its use are available on the OGCA web site at http://main.uab.edu/show.asp?durki=32478.
