As a courtesy, UAB Oral and Facial Surgery accepts insurance payments on behalf of patients receiving treatment. We are providers for most medical and dental insurance plans. However, the payment for services rendered to an insured patient is still the responsibility of the patient and not the insurance company. Some insurance plans require referrals from your primary care physician. Patients are required to have any necessary referrals prior to their visit.

Patients are responsible for all co-payments, partially covered, and non-covered services at the time of visit. If you do not have any medical or dental insurance, payment in full is expected at the time of visit. Financial assistance is available for any medically necessary procedures not covered by a policy. Please call one of the numbers listed below prior to your visit if you have any billing-related questions.

Patients are required to present their medical and dental insurance cards as well as their driver's license.
