The following forms have been pre-approved and are strongly recommended for use in Service-Learning Courses:
Service-Learning Agreement Form (PDF), which is a basic contract between the student and the service agency
Community Partner/Student Mutual Mid-Semester Assessment (PDF) gives the Community Partner an opportunity to evaluate and assist the student mid-semester to maximize and the service experience
Community Partner Final Evaluation of Student (PDF) will form part of the faculty evaluation of the student's engagement with Community Partner

Student Evaluation of Community Partner will assist faculty and Office of Service-Learning to evaluate suitability of service learning sites
Time Log will help students to keep a log of hours worked and activities completed
Weekly Journal Template will help students to reflect on Service-Learning experiences
