All full-time students in the Schools of Medicine, Dentistry, Public Health, Optometry, Nursing, Health Related Professions, all international students and many graduate students are required to subscribe to the Student Health Plan.
All other undergraduate and graduate students are strongly urged to subscribe to the student health plan, but your subscription is optional
Undergraduate students may elect to participate in the Student Health Plan by paying the Student Health Fee at Student Health Services or by sending in their Optional Enrollment Card with a personal check to: UAB Student Health Services, 930 20th Street South, Suite 221, Birmingham, Alabama, 35294-2042. You must fill out and sign this enrollment form (available from the Student Health Service office). See Optional Enrollment for more information.

The Jefferson County Department of Health Clinic is located just two blocks north of our Hill University Center and is available Tuesday-Friday from 8:00am-3:00pm for walk-in immunization and tuberculin skin testing.
