Admission to UAB is the first step to receiving your VA educational benefits. Once you have applied for admission and have been accepted, you can begin the VA process.
If you are a veteran, reservist, or guardsman who wishes to use your federal educational benefits, you must complete VA Form 22-1990 or 22-1995. The U.S. Department of Veterans Affairs requires that you submit the "member" 4 copy and all DD214s and/or DD Form 2384 (NOBE) Notice of Basic Eligibility. If applicable, complete Form 22-686c-Declaration of Status of Dependents. You may access these forms by visiting the G.I. Bill Web site, the local Department of Veterans Affairs, or UAB Veterans Affairs.
Dependents of a veteran who is 100 percent permanently disabled from a service-related illness or injury or dependents of a veteran who dies as a result of his military injuries or illness will need to complete Form 22-5490 or 22-5495.

Once you have registered for courses, please notify us of your intent to use benefits by completing the Veterans Services Benefit Request form on the semester certification page.
Veterans applying for other types of VA benefits, Alabama G.I. Dependents Scholarship Program, Tuition Assistance or ANGAP, will need to contact other UAB offices (see list below).
