You can complete these forms online before printing and signing (when needed), or print and fill in by hand.
Most are also available in the Hill University Center by the Registrar's window.
All forms should be returned to the Registrar's Office, HUC 250.
Emailed forms are not acceptable.

Change of address or name
You may change your local mailing address through BlazerNET. All permanent address changes must be completed in person at the Registrar's Office, HUC 207. Please have photo identification.
Change of Major
To change your major, please log in to BlazerNET with your BlazerID and strong password. Click on the Student Resources tab, then Banner Self-Service, then Student, then Student Records, then "Change Your Major".
Be sure to read the guidelines before attempting to change your major. Certain schools and student populations have restrictions on major changes.
Change of student number
You can change from your SSN to a UAB-assigned number, or the other way round.
Please note: You cannot reverse this process if you change your mind.
If you have lost your original diploma, or need another copy, please submit this form with the fee to order a duplicate diploma.
If you require an enrollment verification for a future term, you may use this form. This is currently an overnight process. The letters can be mailed, faxed or picked up after one business day.
To view and print enrollment or degree verification requests immediately, please log in to BlazerNET
. Click on the Student Resources tab, then Banner Self-Service, then Student, then Registration, then Clearinghouse Student Self-Service Site. You will be directed to the NCH website with further instructions.
The new Forgiveness Policy began October 2, 2006 and replaced the original repeat policy. You may now "forgive" up to 4 different UAB courses in which you gained a grade of C, D, or F.

UAB students must satisfy requirements, including immunization requirements, in order to be medically cleared for enrollment.
If you do not want your contact information to appear in either the printed or online directory, or you require a total hold on all requests not originating from you, use this form.
Please read the form carefully as submitting an incorrect may cause you problems in future.
Transient Form and Letter of Good Standing
To take classes at another school while enrolled at UAB, you must complete this form, acquire the necessary signatures and return to HUC 118.
A Letter of Good Standing may give you discounts on insurance or other benefits.
If you do not complete and submit this form, it will affect your UAB enrollment.
