Learning Disability

Documentation Guidelines

All reports should be typed on letterhead, dated, and signed. Documentation should be no more than 3 years old.  The following should be included in the report:

 

I. Student information including name, date of most recent evaluation

II. Evaluator name, title, and credentials

III. History to Support Diagnosis

a. Description of the duration and severity of the disorder
b. Relevant medical, familial, and social histories
c. Brief description of academic history and evidence of impairment

IV. Evidence of current impairment

a. Description of how disability currently impacts the student in social, academic, or occupational settings
b. Description of how the disability will limit the student in the classroom 

V. Assessment Information

a. Results and interpretation of any clinical interviews conducted
b. Results and interpretation of any testing conducted including scores and subscores
c. Evaluation instruments conducted should include comprehensive intelligence battery and a comprehensive achievement battery normed on adults
d. Examples of instruments measuring aptitude/cognitive ability could include: Wechsler Adult Intelligence Scale IV (or latest version), Woodcock-Johnson Psychoeducational Battery-Revised: Tests of Cognitive Ability, Stanford-Binet Intelligence Scale
e. Examples of instruments measuring academic achievement could include: Woodcock-Johnson Psychoeducational Batter-Revised: Tests of Achievement, Wechsler Individual Achievement Test, Stanford Test of Academic Skills) 

VI. Diagnosis

Clear statement of diagnosis including the DSM-IV-TR 

VII. Recommended Accommodations

Include specific recommendations for academic accommodations and the rationale for the accommodations
