Step 2: Attach the following information to the three above referenced forms:
A photograph (will not be returned)
Copy of most recent school transcript with GPA
UAB acceptance letter or current UAB registration
Photocopy of proof of health insurance (front and back side of card)Step 3: Bring all required items to the first night of clinic.
You will NOT be allowed to participate or try out if you do not have the forms completed. DO NOT MAIL required forms.
