On January 2, 2009, UAB launched a new online process for faculty and staff to disclose external or internal consulting activities and sponsored research submissions. Online forms are now available for submission from three access points: www.uab.edu\provost, http://main.uab.edu/show.asp?durki=30258 (CIRB website) or the BlazerNet Employee Resources tab under "Policies and Forms." Users log in with their BlazerID and password to submit the form.
This disclosure is required from all UAB regular faculty and all staff at grade 20 or above. To streamline the process for complying with disclosure requirements for these activities, all current Provost, school, unit, and Conflict of Interest Review Board (CIRB) disclosure forms have been consolidated and simplified for electronic submission, review, and approval.
The new disclosure process allows faculty and staff to submit information quickly and easily, obtain real-time guidance for interpreting disclosure requirements, and receive electronic responses from appropriate managers through an automated routing process with built-in submission and approval workflows. A simple "smart form" featuring "pop-up" boxes to guide users through submission replaces a process that formerly took 7 separate paper forms. Completed online disclosures are automatically emailed to appropriate approvers upon submission. Disclosers receive email notification when a disclosure is approved and may check the status of a disclosure process at any time by logging in and viewing a history screen.
The online disclosure tutorial gives instructions for completing the form, examples of various activities requiring disclosure, common compliance requirements, and answers to frequently asked questions. In addition, the Office of the Provost and the CIRB will offer a series of information and training sessions at schools and units across campus.
