Rehab Our primary service is patient care, and Physical Medicine & Rehabilitation is a "team" approach to providing care. Each discipline takes an active role in the patient’s care plan. The Patient Services Directory will provide a complete list of in-patient and out-patient services.

For in-patients at Spain Rehab Center, a physiatrist directs the acute medical needs of patients and leads an Interdisciplinary Treatment Team to guide a patient's care plan, which is individually designed to optimize functional potential. Members of that patient’s treatment team is determined by the patient's needs, but Team members usually include rehabilitation nursing, physical therapy, occupational therapy, psychology and neuropsychology, social services, case management, and therapeutic recreation. Because family participation is also a vital part of the rehabilitation process, each patient’s family is invited to attend therapy education sessions so they will be prepared to care for the patient once they return home. Finally, the Team recommends necessary follow-up appointments with specialists for continued out-patient rehabilitation after discharge.

Out-Patient treatment mirrors that of in-patient care. Patients discharged from inpatient rehabilitation can continue their healing process through care from our out-patient clinics and services. Once patients reach their rehabilitation goal, patients are also encouraged to continue regular follow-up in out-patient clinics to prevent or manage potential medical issues that can develop in a person’s lifetime.
