Student Health and Insurance Programs

STUDENT HEALTH SERVICES
Student Health Services provides medical services to UAB students who are enrolled in the program. Student Health emphasizes preventative medicine and provides information on medical matters of interest to students. The service maintains a professional relationship with the student and the strictest confidentiality is maintained.

Participation in the Health Insurance Program is required for:
• all international students,
• students in the Schools of Medicine, Dentistry, Joint Health Sciences, Nursing, Optometry, Health Related Professions, Public Health, and
• students in Engineering, Biology, Chemistry, Physics, and Psychology.

These students will be automatically enrolled in the Health Insurance program unless they submit a waiver to Student Health Services.

Participation in Student Health Services is optional for all other graduate students. To participate in the Student Health Service, students must have an acceptable hospitalization insurance program and must complete a Health History Form.

HOSPITAL and MAJOR MEDICAL INSURANCE PROGRAMS
Hospitalization and major medical insurance coverage is available at attractive rates to students enrolled in the Student Health Services. Spouses and children of eligible graduate students may also obtain coverage under this policy at a proportionate increase in premium. Students who are already covered by hospitalization insurance that offers coverage equivalent to that in the UAB student hospitalization policy may sign a waiver to this effect and not be required to take the UAB policy. Failure to sign and file a waiver with the Student Health Service will result in automatic coverage with Student Health Service insurance and the student will be billed.

STUDENT DENTAL HEALTH PROGRAM
A Student Dental Health Program is available to students enrolled in the schools of Dentistry, Medicine, Nursing, and Optometry. For more information, visit Room 245 of the School of Dentistry at 1919 7th Avenue South or call 934-5234 after noon.
