Financial Assistance

Financial need should not prevent any patient from being seen at CPODD. Financial assistance is available to accomodate for:

• transportation
  • gas/mileage reimbursement
  • KidOne Transportation
  • Taxi service
  • GivingFlite air transportation
• meal and lodging needs
  • meal vouchers
  • Ronald McDonald House
  • local hotels
• some medical costs

• ELIGIBILITY
  • Any patient or family expressing need for financial assistance related to patient care in the CPODD clinic, whether they live inside or outside the State of Alabama
  • Limits: family income $5,000 or more per month, per family of four
  • Exceptions: expressed hardship cases despite income limits
  
  
• PROCEDURE
  • Patient or family may apply by phone, in person, or by mail
  • A grant application must then be completed
  • Our social worker reviews application and either qualifies or denies grant
  • Our social worker then informs coordinator and family
  • All approvals are handled by our coordinator and denials are handled by our social worker
  
  
• DISBURSEMENT
  • Approved grants will be handled by our coordinator
  • Requisition for reimbursement
  • Payment mailed to family
