Referrals & Appointments

Patients are usually seen by physician referral. If you believe your child would benefit from CPODD services, we recommend you discuss the referral with your current neurologist or pediatrician. We will take self-referrals, but we'll always want to speak with your child's doctor and review your child's records before we can set up a CPODD appointment.

BEFORE an appointment can be scheduled, we request the following records:

  • clinic notes

  • hospital discharge summaries

  • lab reports, particularly studies from spinal fluid if a spinal tap was performed

  • MRI images, preferably on disk

  • immunization record

After speaking with your child's doctor or after reviewing the records, someone from the CPODD staff will contact you to schedule your visit.
