Project Description: The proposed project involves the creation and maintenance of ann electronic Palliative Care outpatient information database.  Palliative Care outpatients have been completing clinical questionnaires in the course of routine clinical care for approximately one year; the questionnaires assess pain, quality of life, anxiety, depression, and nutrition, and are maintained in patients' medical records.  The database will include patients' responses to these questionnaires.  The database will also include information about patient diagnosis, prescriptions, medical interventions, and basic biometric measures (e.g. weight, blood pressure, pulse).  Demographic information will also be included.  The purpose of the database project is to track and demonstrate palliative and supportive care intervention efficacy, measure patient outcomes, and to describe the outpatient palliative care population.  Outcomes of this project will be descriptive and interventions reports.  Opportunities for students to initiate abstracts for presentation, publication.

Preceptor will provide intern with access to the following: office or desk space; computer and printer; equipment needed to complete project; supplies needed to complete project
