The State of Alabama Ethics Commission requires that UAB employees who meet a certain salary threshold, or whose job includes management of state funds in certain capacities, complete an annual Statement of Economic Interests form.
The Ethics Commission has provided the form and instructions online for accessing and printing. Please see the links below for the forms and instructions.
Once the form is completed, employees should return it to the State of Alabama Ethics Commission, P.O. Box 302300, Montgomery AL 36130-2300, by the due date of April 30, 2007.
